Social Prescribing Link Workers (SPLW) – several posts

Link Workers will provide connections between the GP and community services that can support residents to address their wider health and care needs.  The Link Worker programme is a national initiative from NHS England and NHS Improvement and we are seeking individuals with a background or interest in health, care, public health or voluntary sector services.Closing Date: 20th August 2020   Interview Date: 9th &  10th September 2020
